New Client Form
IT IS VERY IMPORTANT that you complete this form 24 hours prior to your pet’s first appointment so that we can expedite your check-in and offer the best possible care to your pet.
Previous Medical Records
In order to ensure our healthcare team has sufficient time to review your pet’s medical records to provide the best care, we ask that we receive them no later than 24 hours prior to the appointment. You can drop them off to the practice, email us, or upload them below.
Please read our hospital policies carefully prior to your pet’s first appointment so that you are comfortable with Hill & Harbour Veterinary Center’s expectations. These hospital policies are in place to ensure an outstanding service experience for our clients and the best medical care for our patients.
Prevention Plus Club
Our Prevention Plus Club is an annual wellness plan designed to keep your pet happy and healthy, all while saving you money and giving you peace of mind.
The greatest compliment that we can receive is for you to recommend us to your friends and family!
- Simply go to the Loyalty section of our app.
- Tap Earn More Paws.
- Find the Refer a Friend stamp, and enter your friend’s name and email address.
Your friend will receive a special invitation saying that you think they’ll really love our hospital, and when they come in for their first visit, you will BOTH receive 2 loyalty paws!
We are so excited to offer our Loyalty Program as a way to thank you for being such a valued client and also provide an easy way for you to earn rewards while caring for your pet!
To get started, go to the Loyalty section of your app (ribbon icon) and select Earn More Paws. This will show you all of the ways that you can earn paws at our hospital and then exchange them for rewards.
